Instructions:

1.	Indicate the date and time you wish to take the exam with a check mark. Please Note: Due to limited seating, if the 8:30 a.m. time is full, you will be assigned a later time.
2.	Make check or money order for $25.00 payable to: SDSU TEST OFFICE (DO NOT SEND CASH). The student's name and red ID must appear on the check. Forms with incorrect amount will be returned. Tickets are non-refundable.
3.	Mail this form and your check or money order to: TEST OFFICE Student Services West 2549 SAN DIEGO STATE UNIVERSITY SAN DIEGO, CA 92182-7449
4.	This reservation form must be POSTMARKED by the deadline date. A form received too late to process for the requested date, or with the incorrect amount enclosed, will be returned. Please allow enough time for mailing.
5.	A confirmation ticket will be mailed to you. REMEMBER, you must provide your ticket of admission and the proper ID to be admitted to the exam.
