| 1. | Indicate the date and time you wish to take the exam with a check mark. |
| 2. | Make check or money order for $20.00 payable to: SDSU TEST OFFICE (DO NOT SEND CASH). The student's name and red ID# must appear on the check. Forms with incorrect amount will be returned. |
| 3. | Mail this form and your check or money order to: SAN DIEGO STATE UNIVERSITY TEST OFFICE SS 2549 SAN DIEGO, CA 92182-7449 |
| 4. | This reservation form must be POSTMARKED by the deadline date. Please allow enough time for mailing. |
| 5. | A confirmation ticket will be mailed to you. REMEMBER, you must provide your ticket of admission and the proper ID to be admitted to the exam. For a Math Sample Test, enclose a STAMPED self-addressed envelope. |
