Instructions:

1.	Indicate the date and time you wish to take the exam with a check mark.
2.	Make check or money order for $20.00 payable to: SDSU TEST OFFICE (DO NOT SEND CASH). The student's name and red ID# must appear on the check. Forms with incorrect amount will be returned. Tickets purchased are non-refundable.
3.	Mail this form and your check or money order to: SAN DIEGO STATE UNIVERSITY TEST OFFICE SS 2549 SAN DIEGO, CA 92182-7449
4.	This reservation form must be POSTMARKED by the deadline date. A form received too late to process for the requested date, or with the incorrect amount enclosed, will be returned. Please allow enough time for mailing.
5.	Once your reservation form has been processed, you can print your Ticket of Admission from this website. Remember, you must provide your Ticket of Admission and proper ID to be admitted to the exam.
