For Prospective Students

Apply to SDS

In order to determine eligibility for services and become an SDS student, you must successfully complete each of the following steps:

Step 1:

All students must complete and submit an Application for Services.

Step 2:

Students with learning disabilities

must also complete and submit a Disability Questionnaire and provide a Disability Verification form (contains a section that must be completed by a physician or other appropriate professional).  Appropriate testing documentation must also be provided:

a) Standard scores or compuscores from the Woodcock-Johnson Psycho-Educational Battery-Revised, the Woodcock-Johnson III or IV, and/or scores from the Wechsler Adult Intelligence Scale III.

b) When available, information provided should include an evaluation/report that indicates a specific diagnosis of disability.

Students with ADHD, ABI, or a psychological disorder

must also provide a Disability Verification form.   Depending on your disability, you may be requested to submit additional documentation that will assist us in authorizing appropriate services.

Students with hearing impairments

must also provide a Disability Verification (Physical) form and an audiological report completed within one year prior to the date of application to SDSU.

Students with disabilities not mentioned above

must also provide a Disability Verification (Physical) form.

**Applications and documentation can be faxed, mailed, or hand delivered to the SDS office. The review process can take up to a few weeks, depending on the number of applications that have been submitted.  You may be contacted if additional documentation is needed to determine eligibility.

Step 3

Once eligibility has been determined, you will be contacted to schedule a meeting with an assigned SDS Advisor for an Intake/Information appointment.  Your intake appointment will review needed accommodations and other services for which you qualify.  During this meeting, you will also receive your AAA form (Authorized Academic Accommodations), which lists your approved accommodations and can be used when discussing your academic needs with an instructor. 

Step 4

After your intake appointment, complete the orientation to receive SDS services. 

Step 5

Once you have successfully completed your orientation, you are ready to begin using your approved accommodations and services.  Please remember to follow specific guidelines for the use of authorized accommodations and services.
