To open and read these forms, you will need a copy of the free Acrobat Reader available from Adobe's Web site. If you have Reader, but find you are having problems, go to Adobe's Web site and download the latest version of Reader.

Consent to Disclose Financial Aid Information

A donor for an outside scholarship may request information about you from the Office of Financial Aid and Scholarship (OFAS).  By federal law, we are unable to provide student information to anyone outside of the university without your consent.  For the OFAS to provide any information to a donor, you must complete a Consent to Disclose Financial Aid Information form.

Special Circumstances Petition

If you have been selected to receive an SDSU Scholarship and you will not be enrolled full time and/or not enrolled for the full academic year, complete a Special Circumstance Petition and submit it to the Office of Financial Aid and Scholarships. 

You must already have been selected to receive an SDSU scholarship to use this form.
