Students: Sometimes additional documents and forms are required in order to complete your application file.  These forms are available only through your account on AidLink where you will find detailed instructions describing what you need to do.  Therefore,
check AidLink regularly.  

Available Forms

Carefully read the instructions on all forms before completing and submitting them to the Office of Financial Aid and Scholarships (OFAS). Your request will be delayed if you fail to follow instructions or submit incomplete forms.

Academic Approval Form
— To receive financial aid as a visitor to another institution, an SDSU Academic Adviser must approve the courses you plan to enroll in at the host institution. (You must also submit a completed Special Enrollment Agreement.)

Concurrent Enrollment Agreement for Summer 2008
— To request that units you are taking at another institution be counted towards your financial aid eligibility at SDSU.

Education Abroad Special Enrollment Agreement
— For financial aid recipients on an approved study abroad program through the Education Abroad Office during fall and/or spring semester. Student's enrollment must be through an accredited U.S. college or university other than SDSU or the SDSU College of Extended Studies.

Maximum Units/Semesters Information and Appeal Form
— To request a review of your satisfactory academic progress because you have reached the maximum units and/or semesters allowed for your degree or credential program.

— To request that your financial aid record be updated if your name has changed.
» Important:
You must also update your name with the Office of the Registrar so that it appears correctly on your permanent SDSU academic record. The above form can only be used to update your name with the Office of Financial Aid and Scholarships.

Special Enrollment Agreement
— For classified graduate students enrolled in Thesis Extension 799B through the SDSU College of Extended Studies, and
— For financial aid recipients enrolled as a visitor at another institution,
* or enrolled in nontraditional or external degree programs through SDSU including, but not limited to, the following programs—

  • Executive MBA
  • Master of Science in Regulatory Affairs
  • Sports Business Management MBA

*If a visitor to another institution, you must also download and print the Academic Approval Form.

Study Abroad Program Agreement 2008 - 2009
— For financial aid recipients participating in an SDSU approved study abroad program during the fall and/or spring semesters.

Summer 2008 Federal Work Study Eligibility and Application Procedures
— To be considered for Federal Work Study funds for the summer term

Unit Deficiency Information and Appeal Form
— To request a review of your satisfactory academic progress because you are deficient units.

Verification of Enrollment (for Doctoral Program Students)
— For Joint Doctoral Students enrolled less than half time at the SDSU Campus, and need to verify enrollment for at least half time for the Joint Doctoral Program School for check disbursement. The student will complete Part I and then submit the Verification of Enrollment to the joint school for completion and signature of Part II. The verification can be faxed to the Office of Financial Aid and Scholarships.

Visitor Program Financial Aid Special Program Agreement
— For students in visitor status transfering to another CSU campus through the California State University Intrasystem Visitor Enrollment program.
