Appeals
— For a review of eligibility due to changed financial circumstances, submit a Request for Review.
— For academic progress (maximum units), submit a Satisfactory Academic Progress Appeal form.

Concurrent Enrollment Agreement

— Complete a Concurrent Enrollment Agreement to request that units you are taking at another institution be counted towards your financial aid eligibility at SDSU.
Important — You must be enrolled at least half time (6 units undergraduate; 5 units graduate) through regular SDSU enrollment to have your concurrent units counted.
Doctoral Program Students, Verification of Enrollment Form
— For Joint Doctoral Students enrolled less than half time at the SDSU campus, and need to verify enrollment for at least half time for the Joint Doctoral Program school for funds disbursement. The student will complete Part I and then submit the Verification of Enrollment to the joint school for completion and signature of Part II. The verification can be faxed to the Office of Financial Aid and Scholarships.

— Use the Satisfactory Academic Progress (SAP) Appeal Form.
Name Change Form
— To request that your financial aid record be updated if your name has changed.
Important: You must also update your name with the Office of the Registrar so that it appears correctly on your permanent SDSU academic record. Contact the Office of the Registrar for their procedure.
Parent PLUS Loan
- 2013-14 Parent PLUS Loan Request will be available after July 2, 2013

— Use the Parent Loan Request Form to request a Parent PLUS Loan.
Important:
- Request the full amount you will need for the entire academic year.
- Increases are limited to one per semester.
- Half of the approved loan amount will be disbursed during the fall semester and one half during the spring.
- A loan requested late in the fall semester or during the winter break will not be disbursed until fall grades have been recorded, the student enrolls for the spring semester, and spring classes begin.
Denied Parent PLUS Loan — Use this form to review your options if you were denied a Parent PLUS Loan due to an adverse credit history.

Satisfactory Academic Progress (SAP) Appeal Form
— The SAP Appeal form is to request a review of your eligibility for financial aid to attend summer term 2013 or the 2013-14 academic year. Use this appeal form if:
- you did not complete 75% of the units you attempted summer, fall, and spring, and/or
- your GPA is below the minimum SAP standard, and/or
- you have completed the maximum units and/or semesters for which you may receive aid for your degree.

— Complete and take this form with you to a scheduled interview with an employer. For summer Federal Work Study employment you must —
- Be an SDSU student either continuing from the spring semester or newly admitted and planning to enroll for the fall semester.
- Have a 2013-14 financial aid award from the Office of Financial Aid and Scholarships.
- Maintain satisfactory academic progress at the end of the spring semester.

Visitor Program Financial Aid Special Program Agreement 2013-14
— For students in visitor status transferring to another CSU campus through the California State University Intrasystem Visitor Enrollment program.
